I understand that Summer Boot Camp payment is CASH ONLY, NON-REFUNDABLE and due by the registration deadline. I further agree not to hold Art Hinkelbein Martial Arts Academy, any of its instructors or volunteers liable for any injuries and/or medical emergencies, including death, that take place in and/or as a result of the Summer Boot Camp Program or on the building premises. I, the Parent or Legal Guardian of the aforementioned Student, understand that risk of injury does exist, accept all responsibility for such risk, and attest that the Student is physically fit to participate in the Summer Boot Camp event.
